Garden Road Seventh-Day Adventist Children's Choir Parental Consent and Responsibilities Agreement
By registering your child for the Garden Road Seventh-Day Adventist Children’s Choir, you acknowledge and agree to the following:
1 | Media Consent
We may take photos or videos of your child during choir activities. These images and recordings could be used for church-related purposes, including but not limited to social media posts, newsletters, or the church website.
By registering your child, you agree to allow your child to be photographed or videotaped during choir activities, and consent to the use of these images for promotional and church-related purposes.
2 | Transportation
It is your responsibility to get your child to and from choir activities. The choir, staff, and volunteers are not responsible for transportation. Please make sure your child is dropped off and picked up on time.
By registering your child, you agree that you are responsible for your child’s transportation to and from all choir activities and that the choir, staff, and volunteers are not liable for any transportation-related matters.
3 | Food Allergies and Snacks
Snacks may be provided during choir activities. It’s your responsibility to ensure your child does not eat anything they can’t safely have due to allergies or dietary restrictions. If your child has special dietary needs or allergies, please let the choir director know before any event with snacks. However, please understand that accommodations may not always be possible.
By registering your child, you agree to take responsibility for your child’s food allergies and dietary restrictions, and understand that accommodations may not always be possible. You also agree to inform the choir director of any allergies or dietary needs your child may have.
4 | Medical Emergencies
In case of an emergency where your child needs medical care and you can’t be reached, you give permission for the choir director or another responsible adult to seek medical treatment for your child. It’s your responsibility to inform the choir director of any important medical conditions or needs your child may have.
By registering your child, you agree to give permission for the choir director or a designated adult to seek emergency medical treatment for your child if necessary and if you cannot be reached. You also agree to provide the choir director with any important medical information about your child.
